Social Prescribing for Older Adults (60+ years) bridges the gap between clinical and social care, connecting you to local, non-clinical services chosen to support your wellness, interests and goals. This could be connecting to a walking or exercise group, meeting others who like to play cards, learning how to access food supports, finding a volunteering opportunity and much more. Visit website or call 250-748-2133.
For more information, or to set up a first appointment with Cowichan Hospice, please call us at 250-701-4242 or email frontdesk@cowichanhospice.org.
